Select and print the appropriate account documents from the choices below.  Please note that based on the information that you initially provide, you may be required to complete additional documentation as requested by the FCM.

Individual (includes Individual, Joint, Sole Proprietorship and IRA accounts)

Corporate (includes Corporations, LLC's, Partnerships, Trusts and Pension Plan accounts)

Step 3

Print and sign the additional CFOS documents listed below.  Please note that the Arbitration Agreement below is optional and is not required to open an account with CFOS.

Once your account has been approved, your account is ready to be funded.  You may either enclose a check made payable to "Man Financial" with your account documents or you may also fund by following these wire transfer instructions.
